Our in-home rehabilitation programs utilize nurses, rehabilitation counselors and rehabilitation therapists to provide the following services:

Total Joint Rehabilitation

Our Total Joint Program, with rehab protocols prepared by a surgeon, enables you to begin treatment and get on the road to recovery within 24 hours of your discharge from the hospital. We offer a full continuum of care, including a personalized pre-surgery education and fitness plan, post-acute, home care and outpatient rehab, as well as an updated fitness program for life. Please call 860-658-3783 to speak with Sue Berg to arrange for your personalized program.

Specialty Rehabilitation Programs

McLean Home Care offers specifically tailored, leading-edge therapy programs after an injury, illness or hospital stay to expedite your recovery and help keep you out of the hospital. Our specialty rehab services include:

Home Safety Evaluation and Training

We provide a comprehensive assessment of all risk factors and potential hazards in the home, as well as education on how to stay safe from harm.

Reconditioning and Strengthening

Regaining your strength and stamina after an illness, injury or hospitalization is an important aspect of the total recovery process and provides both physical and emotional benefits. Our therapy team members will demonstrate the appropriate exercises and modifications for each of our specialized patient rehabilitation programs.

Low Vision Training in the Home

McLean Home Care offers a low vision program designed to evaluate, treat and educate low vision patients in the comfort of their own homes. The overall objective of our low vision program is to keep the patient safe and functional in his or her own living environment for as long as possible.

McLean Home Care’s in-home rehabilitation services can help you or a loved one regain the health, functional skills and independence necessary to participate in normal daily activities — whether you are recovering from surgery, an illness, an injury or dealing with total joint rehabilitation, chronic pulmonary or cardiovascular disorders. Recognizing that some patients recover better at home, our services are available to anyone who prefers to have outpatient rehabilitation services in the convenience, comfort and safety of his or her home setting.
